An unusual book, one that will be extremely helpful to genealogists. Gathered here are birth, marriage, and death record application forms from every state in the Union, from U.S. trust territories, and from Canada, Ireland, and the United Kingdom. The forms can be used by anyone seeking copies of these documents for their family histories. The user simply photocopies the appropriate form from this book, then follows the accompanying directions for filing the request with the suitable agency.
