Summary: | Conjoined twins who are classified as craniopagus (joined at the cranium) have a rare congenital anomaly and can be classified into complete or partial, depending on whether or not they have shared dural venous sinuses. The separation of craniopagus is a very rare and complex challenge needing detailed evaluation and planning. The video describes the separation of two sets of total craniopagus twins. A total of 4 multistaged neurosurgeries and one separated plastic surgical procedure for skin expansion were performed. The multistaged surgery reduces complications improving venous drainage, prevents increased venous pressure, diminishes cerebral edema. The different stages also allowed the twins to recover from each surgery before progressing to the next stage, while continuing to receive nutritional support and physical therapy. Successful separation of total craniopagus twins, where both could come out of surgery without any neurological deficit remains a rare occurrence and was our ultimate goal.
